How to register/Updating Your details HOW TO REGISTER If you live in our catchment area (Moor Park, Northwood, Northwood Hills, Pinner Green) please bring your medical card to reception and ask to register with us. If you do not have a medical card you will be asked to complete a registration form with your previous GP’s details if you had one. You will also be asked to complete a new patient questionnaire and make and appointment for a new patient health check with our nurse. You will need to bring the form to the surgery with proof of his/her identity e.g passport, birth certificate, and proof of address i.e a recent utility bill CHANGE OF NAME/ADDRESS Please notify us of any changes as soon as possible so that we can amend our records accordingly. Unfortunately we have to say goodbye to patients moving from our catchment area. Please register with a new GP as soon as possible so that the Health Authority can transfer your records. |
